1. Electronic Visa Application Form DS-156 (EVAF)

Every applicant for a nonimmigrant visa must fill out the DS-156 on-line. Once completed, applicants should print all three pages of the DS-156, including the bar code, for submission to the Embassy. The Embassy will not accept photocopies or faxes of the form.

Instructions for completing an Electronic DS-156:

  • Be sure to answer every question on the application. If a question does not apply to your circumstances, write “N/A”. Your answers should be in English and use English characters. Letters like ñ, é, ü, ç are not recognized by the system.

  • Review the information you entered for accuracy.

  • Press the “continue” button at the end of the form. Note that this process generates a bar code and could take several minutes. Failure to click on the “continue” button results in an incorrectly formatted application form which the Embassy will not accept.

  • Print the completed DS-156 and then date and sign in.

Note that the EVAF program does not allow applicants to save their data – once you exit the EVAF website, all your data disappears.

2. DS-157 Form

All male applicants between the ages of 16 and 45 must also complete a supplementary application form (form DS-157).

3. DS-158 Form

Applicants seeking a F, J or M visa must also complete Form DS-158 Supplemental Contact Information and Work History for Nonimmigrant Visa Applicant.
